The approval request must be made by completing Annex II (official request form) and sending it to the Directorate of the Social Marine Institute; this document must be submitted along with the additional documentation indicated below to the central services of the Social Marine Institute, at its provincial headquarters or local centres, or sending it by any means permitted under article 38.4 of Law 30/1992, of 26 November, of the Legal Regime of Public Administrations and the Common Administrative Procedure.
One request must be submitted for each modality or training subtype that the centre wishes to teach; requests must be accompanied by the following documentation:
a) Accreditation of the identity of the applicant and/or legal representative. For the purposes of identification, the applicant must present:
- For individuals: the approval form (Annex III) duly filled in for the purposes of corroborating data using the IT system, with the corresponding confidentiality guarantees. Failing this, an attested photocopy of his/her national identity document, passport or NIE (foreigner's identification number).
- For legal entities: certified photocopy of the beneficiary's tax identification number.
- Supporting documentation for the legal representative, where applicable. This may be replaced by certification issued by the corresponding provincial headquarters of the Social Marine Institute.
b) Articles of association, for legal entities.
c) Certificate demonstrating that the requester has no outstanding tax and Social Security payments. No such certificates will need to be presented by the requester if he/she consents to said documents being collected by the management body of the State Treasury Administration Department and the Social Security Treasury General.
d) Certificate of the quality system implemented, issued by an external auditor, which includes the provision of specialist health training.
e) Report describing the points specified in Annex VI of the Resolution of 24 April 2013:
- Facilities, furniture and equipment (only for regular courses):
- Minimum classroom surface area (m2/student).
- Description of shared facilities.
- Description of teaching fittings and auxiliary elements.
- Safety and hygiene equipment on site: fire-extinguishing devices, safety signage, first-aid equipment, habitability of classrooms (heating, air conditioning, lighting, cleaning, etc.).
- Teaching methods and material:
- Description of audiovisual material used in training activities.
- Description of teaching aids.
- General organisation of the centre.
- Maintenance and conservation plan for equipment and materials.
- Training plan (course content and times frames assigned, design of theory and practical teaching units, procedure to be used in theory and practical tests during courses).
- Assessment methods for the training activities.
- Form for reporting on teaching activities.
- Survey form for assessing student satisfaction.
- Human resources.
- Teaching staff.
- Administrative staff.
- Administrative management. Tasks related to administrative control and tracking for the courses provided: filing of academic transcripts and documentation for students, backup copies of computer files, sample master record for certificates and awards granted, sample master records for courses.
f) Declaration of adherence to the minimum conditions set out under annex II of Order PRE/646/2004, of 5 March.
g) A copy of the student handbook, the content of which must be adjusted to the objectives set out under annex I of Order PRE/646/2004, of 5 March.
All documentation will be provided in paper copy and in electronic form.
