You can use this form to apply for registration or deregistration from a special agreement, as long as it is an agreement other than the agreement for non-professional carers, the agreement for people with disabilities and special difficulties in finding employment, the special agreement for internships or the special ERE agreement, which have a specific application form. You can also use this form to request a change in the details of your existing special agreement.
If you have an electronic certificate, DNIe or permanent Cl@ve, you can access the Social Security E-Office and, through the procedure Registration in special agreement, Variation of special agreement data or Cancellation of special agreement, submit your application online by filling in the application electronically.
If you do not have these means of identification, you may send an application, selecting the category "Special agreements" and choosing the corresponding procedure according to the type of special agreement whose registration, variation of data or deregistration you are requesting, where you may attach the completed application form.
