You will need to apply for the benefit from the National Social Security Institute, using the forms shown below, as long as your company is insured for occupational risk with this Management Entity. If the insurance is with a Mutual Collaborating Insurance Company, you must apply to them and they will pay the benefit at their own expense.
If you are a seafarer and your company has occupational risk insurance with the Social Marine Institute, you must submit your application to this Management Entity using the form for the Special Scheme for Seafarers.
