Original together with a copy for authentification or with an already authentificated photocopy, except  for identity documents, where showing the original will suffice

 

IN ALL CASES

 

1.Proof of the applicant's identity by means of the following valid documentation:

  • Spanish citizens: DNI 
  • Foreign citizens: Passport or, where appropriate, identity document in force in their country and NIE required by the National Tax Administration Agency for payments.

2. Documentation related to contributions:

  • Workers employed by another person in the General Scheme: Certification from the  last company/companies where they worked.
  • Workers employed by another person in the Special Agricultural Scheme: Company certificate correctly filled in according to the TC 2/8 for the month prior to the start of the work contract's being suspended and proof of salary for the last three months (TC 1/9).
  • Workers who are self-employed: Receipts for contribution payments for the last three months.
  • For Artists and Professional Bullfighters declaration of activities (TC 4/6) and receipts for performances (TC 4/5) that have not be sent to the Treasury General of the Social Security.
  • Sales Representatives: Contribution payment receipts for the last three months (TC 1/3).

3. Medical Information:

  • Medical certificate of sick leave and confirmation of sick leave.
  • If the worker comes under a delegated payment option, confirmation report(s) following the last one filed by the employer.
  • If medical discharge has occurred, medical certificate of the discharge.

4. Report on work-related injuries or occupational diseases (WRI  and OD ), filled in by the company, where appropriate.

DOCUMENTS ACCREDITING THE TERMINATION OF EMPLOYMENT:

5. If the termination occurs in the first  15 days of temporary disability: work contract and extensions.

6. In cases of dismissal: dismissal letter, record of conciliation or sentence.

OTHER DOCUMENTATION:

7. Declaration of activity status, only for workers in the Special Self-employed Workers Scheme.

8. For workers in the Special Agricultural Scheme who are employed by another person: Proof of actual days worked.

9. Family Record Book and certificate of disability stating the certified degree of disability, in cases of termination of employment, and the children to take into account when applying contributory unemployment limits.

10. Companies with fewer than ten workers: it is necessary to report this at least 15 days prior to the application so that the company can change its obligation to pay directly to the INSS  starting from the 1st  day of the following calendar month.


This documentation must be presented at any of the Social Security's Service and Information Centres.
