Data that must be included in the applications

Notwithstanding each regime's specificities, in general, the affiliation application shall contain data related to the performance of the activity in order to provide the General Treasury of the Social Security with complete information, and in particular:

  • Name or corporate name of the employer promoting the affiliation.
  • Employer's Contribution Account Code.
  • Social Security Regime.
  • Worker's name and surnames.
  • Worker's Social Security Number.
  • National ID 
  • Worker's address.
  • Date the activity started.
  • Contribution Group. 
  • AT  and EP (acronym in Spanish for Work-Related Injury -W.R.I.- and Occupational Disease -O.D.-) (only for registration before 31-12-2006) heading
  • Type of contract and working day coefficient, if applicable.
  • Work (in the indicated cases of Additional Provision Four of Law 42/2006 28 December 2006)

In the case of applications for termination of affiliation, they must contain the data relative to:

  • Name or corporate name of the employer instigating the termination of affiliation.
  • Employer's Contribution Account Code.
  • Social Security Regime. 
  • Worker's name and surnames. 
  • Worker's Social Security Number. 
  • National ID 
  • Worker's address. 
  • Date of termination of activity 
  • Reason for termination of affiliation.

The data change application, in addition to the worker's and employer's identification data, shall contain the data subject to modification, with the requirement to notify, primarily, those changes affecting working conditions:

  • Type of contract and working day coefficient.
  • W.R.I and O.D. (for periods after 31-12-2006)
  • Change in Trading Group.
  • Date of change.
  • Work.

In the case of self-employed workers, in addition to the data specified above, it shall include the data related to:

  • Professional activity.
  • Place activity is carried out.
  • Social Security Regime.
  • Any peculiarities regarding contributions and protective action
